Situation:
The Transitions program is a community outreach tool that provides practical information for families taking increasing responsibility for the health and well-being of aging parents. Through a series of seminars, family members are coached to manage or prepare for the medical, residential, social, financial, legal and emotional changes that accompany aging. The comprehensive program tool kit makes the turnkey seminars easy to promote and deliver through churches, community groups and employers.
